Failure mode and effects analysis (FMEA) assesses the potential risks of system failure and proposes a plan of detection, prevention and risk mitigation for identifiable failure effects. The method provides valuable insight on what might go wrong with a product or process. Different types of FMEA such as Design Failure Mode and Effects Analysis (DFMEA) and Process Failure Mode Effects Analysis (PFMEA) exist to evaluate risk in different stages or inputs of the manufacturing process.
Use this template to systematically evaluate potential failures:
- Analyze failures across the 3 parameters of severity (impact of the failure), occurrence (failure cause and frequency), and detection (likelihood of failure detection) to calculate a RPN (risk priority number)
- Set contingency plans with corrective actions
- Prevent failures and mitigate risks
